The Midwestern Vascular Surgical Society invites you to participate in Midwestern Vascular 2009, the
Society’s 33rd Annual Meeting to be held September 10-12, 2009 at the Westin Michigan Avenue
in Chicago.

Our Chicago meetings have excellent attendance with approximately 200 vascular specialists in
attendance. These specialists include members and guest physicians, vascular nurses, technicians,
physician assistants, and vascular fellows in training. The Society will continue to integrate the
endovascular course (New Horizons in Vascular Surgery) with the main meeting, and will continue this
format of broad coverage of both open and endovascular topics throughout the meeting.

The fee for exhibiting will be $2,500 for a table-top display. The Society also is excited and pleased to
offer Companies the opportunity to participate as educational sponsors with educational grants in the
amount of $5,000 and $7,500 (these amounts do not include the exhibit fees). Please contact the Society
for further information.

When planning your participation, please consider the following:

• The meeting will feature an exciting educational session that will include new innovations and
techniques in endovascular and vascular surgery which will be integrated with the abstract
presentations. The program will be accredited for Continuing Medical Education Credits.

• Attendees have the opportunity to visit your company’s exhibit during prime exhibit hours in the
morning and through the day that includes a Welcome Reception in the Exhibit Hall on Thursday,
September 10th. Additionally, we encourage all company representatives to attend the Friday evening
event that will be held at the Westin Michigan Avenue.

• The Society will continue to offer the Passport program and giveaways to physicians who visit all
exhibits during the meeting.

• 25% of the physicians who attend the Meeting are in private practice with the remainder coming
from major teaching institutions and clinics. 95% of the physicians who attend have specialties in
general, vascular and endovascular surgery.
